At first blush, this sounds fairly trivial, but Male and females does not cover all genders. There is a section of the population that could be treated as "Transgender". These could be Males that are becoming female, Females that are becoming male, persons with both male and female attributes.
In addition, there is a sector that would prefer not to give their gender (strange but true)

For medical purposes, only the gender at birth is sometimes relevant in determining the proper treatment. Other than that, they could use 'Male', 'Female', 'Male (transgender)', 'Female (transgender)' and 'Not specified'.

Since the South African Identity Number is required, gender can be inferred anyway as it's encoded in the identity number.
Format: yymmddssssnec
yy = year
mm = month
dd = day
ssss = male if >= 5000, female if < 5000
n = born in South Africa (0 for yes, 1 for no)
e = ethnicity - not used anymore
c = check digit to verify the identity number is in a valid format and has not been tampered with.
